The PhilHealth ID is an identification card issued by the Philippine Health Insurance Corporation for its members. The ID card serves as an important identification that demonstrates your membership. It qualifies you as the cardholder and your qualified dependents for medical services and PhilHealth benefits. You may present the card when dealing with PhilHealth to expedite the processing of claiming any PhilHealth benefits and other transactions.
There are two types of PhilHealth ID cards you can apply for: the regular PhilHealth ID card and the digitalized PhilHealth Insurance ID card. There are several differences between the two PhilHealth ID cards.
The regular PhilHealth ID card is a paper-based card. This type is issued for free to all PhilHealth members. Since this paper card has no security features, it cannot be considered a valid ID. At the time being, only the regular PhilHealth ID paper card can be obtained from PhilHealth.
The PhilHealth Insurance ID card is a more advanced digitized plastic card that has a QR code and magnetic stripe. It contains more personal information than the traditional regular PhilHealth ID card. This type is optional for employed members. This health insurance ID costs PHP 90 for three-year validity. This card can be considered as a valid ID.
The front portion of the PhilHealth ID carries the basic information of the cardholder, including full name, PhilHealth Identification Number (PIN), signature, home address (for the digitized card only), a space for 1 x 1 PhilHealth ID picture, and the validity period of coverage. The back side contains a certification as to the cardholder's identity and entitlement to PhilHealth benefits, along with the signature of the PhilHealth President and CEO.
To apply for a PhilHealth ID, you must prepare the following documents:
A PhilHealth ID picture serves to identify the cardholder. The photo must comply with the following official requirements for a successful PhilHealth ID application.
The size of the photo should be both width and height of 1x1 inch (2.5 x 2.5 cm or 25 x 25 mm). Your head must be between 65 and 68.25% from the bottom of your chin to the top of your head. There should be 1/10 inch from the top of the photo to the top of the hair.
The background should be plain white without any shadows and patterns.
When taking your PhilHealth ID photo, you should pose correctly. You should give a full-face view with a neutral facial expression. Do not smile, frown, or open your mouth. Eyes should be completely open and stare at the camera directly. Head and shoulders should be straight ahead of the camera. By the way, try to be relaxed throughout the whole photo-shooting process.
If you don't want to spend too much time and money on photo studios to take your PhilHealth ID pictures, you can shoot one with your cellphone at home instead. It is highly suggested that your family or friends shoot pictures for you using a smartphone with a high-resolution camera. You will also need a photo editing tool. Before shooting, here are a few preparations:
After you choose the best shot out of the many, you may use a photo editing tool to crop it to an appropriate size.
According to the PhilHealth ID picture specifications, the size of the PhilHealth ID Photo should be 1 x 1 inch. If you want to change the size of your photo to 1 x 1 inch, there are three ways.
Step 1. Access the AiPassportPhotos' website and upload the photo you'd like to crop.
Step 2. Wait for about 15 seconds, it will check if your photo can be used for your PhilHealth ID. If not, it will give you feedback and suggestions for you to reshoot the photo. If applicable, it will process the digital photo and make it compliant.
Step 3. When the photo processing is finished, click "Get photos now" and the digital PhilHealth ID photo template will be sent to your email address. You can print it on high-quality matte or glossy photo paper with a printer. Or you can have it printed in a photo studio nearby.
You should provide two copies of 1x1 inch ID photos when applying for a PhilHealth ID. They should be colored and taken in the last six months.
No, the PhilHealth ID does not have an expiration date. The PhilHealth identification number itself is permanent and unique. If your PhilHealth ID card is lost, damaged, or stolen, you can apply for a replacement of the card at your nearest PhilHealth Office in person. You should provide at least two valid IDs such as a birth certificate and passport.
